CUSTOMER WORK AUTHORIZATION & AGREEMENT
Flooring USA and More, LLC
Customer Work Authorization & Agreement
This Agreement applies to the products, materials, labor, installation, repairs, and other services described in the Customer’s approved estimate, proposal, invoice, or work order. By signing, Customer authorizes Flooring USA and More, LLC (“Company”) to perform the approved work and agrees to the following terms.
1. Scope of Work
The approved estimate or work order defines the project. Services may include flooring, cabinets, countertops, painting, repairs, installation, or other construction-related work.
When multiple departments or types of work are included, each is considered a separate scope of work unless specifically combined in writing.
2. Estimates, Materials & Preparation
Project pricing is based on estimated measurements, quantities, labor, and known site conditions. Estimates are not guaranteed final prices.
Actual material or labor requirements may vary due to field measurements, waste, cuts, product requirements, or site conditions. Reasonable additional materials required to complete the approved work may be added to the final cost.
Additional preparation, including floor leveling, self-leveling, skim coating, subfloor repair, moisture-related work, or other necessary preparation, is an additional charge unless specifically included in the estimate.
3. Changes & Additional Work
Customer-requested changes or unforeseen work may affect the price and completion schedule.
Material changes or additional work will be communicated to Customer and, when reasonably practical, approved by change order, revised estimate, invoice, email, text, or other written/electronic authorization before being performed.
4. Customer-Supplied Materials
Customer is responsible for the suitability, accuracy, availability, condition, and quantity of Customer-supplied products or materials.
Company is not responsible for product defects, shortages, incorrect materials, delays, manufacturer issues, or product failures involving Customer-supplied materials. Company’s workmanship warranty still applies to installation performed by Company.
5. Hidden & Existing Conditions
Installation or removal may reveal conditions that could not reasonably be identified beforehand, including moisture, damaged or uneven subfloors, plumbing, electrical, structural, wall, or other concealed conditions.
Additional work or materials required to correct these conditions may result in additional charges.
6. Property & Work Area
Company will exercise reasonable care while performing the work but is not responsible for pre-existing damage, concealed conditions, deteriorated materials, or reasonable effects of performing the contracted work.
Customer is responsible for removing or securing fragile, valuable, or easily damaged property unless other arrangements are made.
Any suspected damage should be reported promptly so Company has an opportunity to inspect it.
7. Jobsite Safety
Children, pets, guests, occupants, and other unauthorized persons must remain reasonably clear of active work areas.
Company tools, equipment, ladders, machinery, and materials may not be used or handled by unauthorized persons. Company is not responsible for injury or damage resulting from unauthorized entry, use of equipment, or interference with work in progress, except where caused by Company’s negligence or otherwise required by law.
8. Completion & Final Payment
A department or scope is substantially complete when the primary contracted work has been completed and can reasonably be used for its intended purpose.
Unless otherwise agreed in writing, the remaining balance is due upon substantial completion. Any unpaid balance becomes past due five (5) business days after substantial completion.
Minor adjustments, punch-list items, or warranty matters do not delay payment of the undisputed balance. Each department or scope may reach completion and become due independently.
9. Workmanship & Manufacturer Warranties
Company provides a 90-day workmanship warranty beginning upon substantial completion. Covered workmanship issues will be inspected and, when determined to be Company’s responsibility, reasonably corrected.
The workmanship warranty does not cover normal wear, misuse, accidents, moisture/water damage, structural movement, product defects, modifications by others, Customer-supplied product failures, or conditions outside Company’s reasonable control.
Manufacturer product warranties are separate and subject to the manufacturer’s terms.
Customer agrees to give Company a reasonable opportunity to inspect and correct a claimed workmanship issue before another contractor performs repairs, except when emergency action is reasonably necessary.
10. Project Photographs
Company may photograph the project for documentation, quality-control, and warranty purposes.
With Customer’s permission, photographs or video showing only the work performed may also be used for Company’s website, social media, portfolio, or advertising. Marketing materials will not intentionally show Customer names, addresses, house numbers, faces, license plates, or other personally identifying information.
Marketing Photo Preference
If YES — Company may use photographs/video of the work for marketing.
If NO — Do not use photographs/video of my project for marketing.
Selecting NO does not prevent photographs needed for internal project or warranty documentation.
11. Project Delays
Company is not responsible for reasonable delays caused by circumstances outside its control, including material availability, shipping, weather, concealed conditions, inspections, utilities, Customer changes, or similar circumstances.
